Heart to Heart Hospice - - Responsibilities: Provide direct patient care to patients under the supervision of the RN and according to the Hospice Aide/Homemaker Plan of Care; Assist the patient with safe transfers and ambulation per agency policy; Report/communicate changes and document pertinent information and care provided to patient to ensure continuity of care; Practice infection control principles and maintain confidentiality; Maintain professional communication with patient, family, and staff; ensure timely documentation and compliance with laws and policies
